QA Lead SNF Liaison LVN
Overview
The QA Lead / SNF Liaison (LVN) is responsible for supporting the operations of the Quality Assurance department while serving as a key point of contact between Skilled Nursing Facilities (SNFs), providers, and our internal team. his is a non-clinical role with no direct patient care. Instead, the position focuses on chart audits, issue resolution, client relationship management, and occasional business development activities. With a Monday–Friday, 8–5 schedule, this role is ideal for Licensed Vocational Nurses (LVNs) who want to leverage their clinical background in a stable, office-based career path while maintaining some client-facing responsibilities through travel.
Essential Job Functions
- Oversee and maintain QA-related documentation, including auditing patient discharge (D/C) charts and conducting clinical audits on active patients.
- Track, manage, and resolve provider/patient issues, maintaining a detailed log of communications and resolutions.
- Act as a liaison between SNFs, providers, and internal teams to ensure smooth operations and strong client relationships.
- Provide timely and professional communication to address provider issues and escalations.
- Collect and analyze data/metrics to support financial analysis, hiring decisions, and organizational growth initiatives.
- Collaborate with the Quality Assurance team to uphold standards of compliance, safety, and documentation
- Support marketing and business development efforts, including relationship-building with new and existing SNFs, and attending site visits
- Assist leadership with QA initiatives, operational improvements, and client engagement strategies.
Qualifications
- Current California LVN license required.
- 1–2 years of experience in a Skilled Nursing Facility (SNF) setting preferred.
- Strong knowledge of healthcare quality standards, compliance, and safety requirements.
- Excellent written and verbal communication skills; ability to manage client-facing interactions with professionalism.
- Strong organizational and analytical abilities; skilled in maintaining accurate records and data tracking.
- Collaborative mindset with proven ability to work across multiple teams.
- Comfort with 20% travel for client visits and business development activities.
